Ex-Employee Support
Support for Ex-Employees and their immediate families is through the purchase of healthcare equipment to allow for a better quality of life and greater independence.
Application for Equipment
In order to apply for healthcare equipment applicants need to have worked in British Enkalon Antrim for more than one year and have a referral from an Occupational Therapist. Enkalon Foundation are not in a position to assess the type and spec of equipment required by an individual so an assessment is required by an occupational therapist. If you are not already registered with an Occupational Therapist you will be required to visit your GP and ask for a referral.
Once you have been assessed by an Occupational Therapist and they have informed you that they will be submitting a referral to us you will be required to submit a letter of request. The letter/email should contain the following details:
Information required when the applicant is an ex-employee:
- Full name and address
- Contact telephone number
- Date of birth
- Approximate year of employment and department worked in
- Healthcare equipment being applied for and reason for application
- Contact telephone number
- Date of birth
- Approximate year of employment and department worked in
- Healthcare equipment being applied for and reason for application
Information required if applicant is a relative of an ex-employee:
- Full name and address
- Contact telephone number
- Date of birth
- Healthcare Equipment being applied for and reason for application
- Full and and address of ex-employee
- Date of birth of ex-employee
- Approximate year of employment and department worked in
Once we have received a referral from an Occupational Therapist and your letter of request a quote for equipment will be sought from Sync Living. Depending on the equipment needed this may require a visit to your house. When the quote has been received the information is sent out to the Trustees for a final decision. Applications for ex-employees and their spouses are continually assessed by the Trustees, relatives beyond spouses i.e siblings, children and grandchildren will be assessed at the Trustees quarterly meetings (March, June, September and December)
